Case Management

At Green Ridge Health Care Center our case management system promotes resident advocacy. Our interdisciplinary wellness team of caregivers provides sound management and recommendations regarding the resident's clinical and financial resources to positively impact quality of life. We strive in our goal-oriented approach to assure that the physical, social, recreational, spiritual, and educational needs of our residents are met. Our wellness team in comprised of:

Physicians specializing in rehabilitative medicine and treating diseases pertaining to the geriatric population.

Professional Nurses To assure that each resident's plan of care is monitored and administered as intended.

Certified Nursing Assitants educated in geriatics who will provide assistance with each resident's activities of daily living (grooming, bathing, dressing, feeding, etc.)

Recreational Activities Therapists to provide individualized activities enjoyable to each resident. Residents enjoy a full and varied activities program designed to meet each resident's individual preference and ability level.

Physical, Occupational, and Speech Therapists to provide a comprehensive medical rehabilitation program which incorporates our various medical specialists and professional staff to develop each resident's maximum potential.

Pharmacists to provide medications and monitoring to assure maximum effect while reducing any food or drug reactions.

Dietician to provide a nutritious, varied menu and assorted snacks including special diets.

Housekeeping and Maintenance Staff to provide a sparkling clean and safe environment.

YOU and your loved ones are the most important members of our team. Our wellness team strives to build relationships with residents by learning about their life experiences and daily routines. This helps the staff members to understand each resident's preferences and can alert them to any indication that the psycho/social status may be changing. An ongoing dialogue between residents, their families, and other team members helps all interested parties make better, more informed decisions.
